Geriatric Care FAQ

  • What exactly is a Geriatric Care Manager?

A Geriatric Care Manager is a professional who guides families through the healthcare, legal, and logistical mazes of aging.

At Nova, we go a step further. Because we are led by a Masters-level Nurse (MSN), we combine the logistics of care coordination with deep clinical expertise.

We act as your advocate, translator, and strategic planner, ensuring your loved one’s quality of life is maximized while reducing family stress.

  • What is the difference between Nova and a home health agency?

    This is a common confusion.

    • Home Health Agencies provide the hands-on staff (caregivers, aides) who assist with things like bathing, dressing, and feeding.

    • Nova, which is a consultancy, provides guidance, oversight, and strategy.

We do not provide hourly caregivers ourselves. Instead, we vet, hire, and manage  them.

We are the architects who design the care plan. The home health agency provides the builders who execute it.

If a caregiver isn't performing well, we step in to fix it so you don't have to.

  • Why do I need a consultant? Can't I just manage this myself?

Many families try to manage care on their own, often leading to "caregiver burnout." You might need Nova if:

    • You live out of town. You need "boots on the ground" in Northern Virginia to be your eyes and ears.

    • The medical situation is complex. Your loved one has multiple diagnoses, conflicting medications, or frequent hospitalizations.

    • Family conflict. Siblings or other family members disagree on the best course of action and need a neutral, clinical third party to mediate.

    • You are overwhelmed. You want to be a son, daughter, or spouse again, rather than a full-time case manager.
  • Why should I hire a nurse (RN, MSN) instead of a social worker?

Aging is often driven by medical conditions. Our advanced nursing background allows us to do things like:

    • Catch medical errors before they become emergencies.

    • Review medications for dangerous interactions.

    • Translate "Doctor Speak" into plain English for the family.

    • Push back on hospital discharge planners when they try to send a patient home too soon.
  • What does "Certified Dementia Practitioner" (CDP) mean for my family?

The CDP credential means we have undergone specific, advanced training in Alzheimer's and dementia care.

We don't just "watch" patients with memory loss—we actively manage their environment to reduce anxiety and behavioral issues.

We can teach you how to communicate effectively with a parent who is confused, preventing the escalation of difficult behaviors.

  • What areas do you serve?

We are a locally focused, boutique practice serving Northern Virginia (NoVA), including:

    • Fairfax County (McLean, Great Falls, Vienna, Reston)

    • Arlington & Alexandria

    • Loudoun County

    • Prince William County

We also offer virtual consultations nationally—and even internationally.

  • Do you attend doctor's appointments?

Yes. One of our most valuable services is accompanying clients to medical appointments.

We ensure the doctor receives accurate history (which patients often hide or forget), ask the tough clinical quesrtions, and provide a written summary of the visit to the family with clear "next steps."

  • Does Medicare or insurance cover your services?

Generally, no. Geriatric Care Management is a private-pay advisory service. Medicare and private health insurance do not currently reimburse for this level of personalized consulting and advocacy.

However, some Long-Term Care Insurance (LTC) policies may cover a portion of care coordination. We can review your policy to help you determine if this benefit is available to you.

  • Can you help with hospital discharges?

Absolutely. Hospital discharge is a critical vulnerability point. We ensure the discharge plan is safe, that the rehabilitation facility is high-quality, and that medication lists are reconciled.

We advocate for your loved one to ensure they aren't pushed out of care prematurely.

  • How do you charge?

As a boutique consultancy, we prioritize transparency.

    • Initial Assessment. A flat fee for a comprehensive review of medical, social, and environmental needs, resulting in a detailed roadmap/care plan.

    • Hourly Consulting. For ongoing support, crisis management, and coordination, we bill at an hourly rate.

Please contact us directly for our current fee schedule.

  • My parent is in the ER right now. Can you help immediately?

Yes. We specialize in crisis intervention. If you are in the middle of a medical emergency, we can deploy to the hospital to interface with the medical team, help you understand your options, and prevent hasty decisions that could have long-term negative impacts.

  • Do you help with moving to Assisted Living?

Yes. The number of facilities in Northern Virginia can be dizzying.

We know which communities have the best nursing staff, which have the best dementia programming, and which have had recent regulatory violations.

We help you select the right fit, not just the one with the nicest lobby.
